At Connect, we celebrate the uniqueness of every individual and design support around what matters most to them. That’s why everyone attending our sessions is guided through a simple person centred Support Plan. This plan helps our staff team to understand more about each of our service user’s needs, abilities, and goals enabling us to provide them with support that allows them to express themselves, feel assured and participate in our activities safely. 

What to expect from our support plans

  • Communication preferences – how do you like to communicate (verbal, visual, gestures, or symbols) any strategies staff can use to support understanding.
     

  • Sensory needs – triggers, calming strategies, and sensory-friendly adaptations to create a comfortable environment.
     

  • Interests and strengths – activities the person enjoys and excels at, to encourage engagement and motivation.
     

  • Social and emotional support – guidance on how to support social interaction, manage emotions, and build confidence in group settings.
     

  • Goals and aspirations – for personal development, social participation, or wellbeing.
     

  • Health and wellbeing considerations – relevant medical needs, mobility support, or routines that promote safety and comfort.
     

  • Preferred routines and structure – daily or weekly schedules, pacing of activities and approaches that help reduce anxiety or overwhelm.
     

Our team of caring staff will work with you, family and relevant professionals to develop a plan that is realistic, achievable and meaningful. The plan is regularly reviewed to ensure it continues to meet the person’s evolving needs and preferences.

Support plans are an essential part of safeguarding because they provide staff with a detailed understanding of each individual’s needs, preferences, and vulnerabilities. Support plans ensure that every interaction is safe, respectful, and appropriate. They help staff anticipate potential risks, respond effectively to concern  and create an environment where our service users can participate confidently and securely.

We collect essential details such as medical needs, allergies, communication preferences, and support requirements to ensure every session is safe and appropriately managed.

Support plans highlight strengths, abilities, and personal goals so we can help service users grow confidence, learn new skills, and take positive steps toward independence.

All staff and volunteers work from the same clear guidance, ensuring continuity and high-quality support at every session.

We capture important information about what helps someone feel calm, supported, and emotionally secure, especially for individuals who experience anxiety, sensory challenges, or communication barriers.

Support plans allow people to express what matters to them — their likes, dislikes, routines, and personal preferences — helping us create an environment where they truly belong.
